In recent decades, the Gavkhoni Basin has faced serious challenges in the management of water resources and uses across various sectors due to factors such as population growth and its disproportionate distribution, the expansion of agricultural activities, the development and concentration of industries, climatic fluctuations, and inefficient management of water resources. The present study focuses on the critical area between the Zayandeh-Rood regulating dam and Nekoabad dam, which Is the main center of water allocation in the basin, and assesses the state of water management and identifies existing challenges. In this study, the comprehensive DPSIR framework (Driving force–Pressure–State–Impact–Response) was employed for an Integrated analysis of the cause-and-effect chain, from the macro level of policymaking to the micro level of exploitation and its consequences. Driving forces and responses were identified and prioritized using the Delphi method, expert opinions, and questionnaires distributed among 45 water specialists and experts. Pressures, state, and impacts were also evaluated using statistical data, reports from the Regional Water Company, remote sensing, and field visits. Findings indicate that the lack of coherent policies and the absence of clear strategies are the main driving forces leading to sectoral actions and conflicts of interest. Through pressures such as land use and land cover change, increased cultivated areas and impervious surfaces, a shift in cropping patterns toward water-intensive crops, and increased industrial uses and water pumping stations, this has directly led to reduced resources and increased water demand. The consequences of this trend, especially in downstream and tail-end areas, have been extensive; among environmental challenges, mention can be made of reduced inflow and outflow volumes and drying of the riverbed during parts of the year, the designation of plains in the Lenjanat area as prohibited for groundwater development, an increase in the number of deep and semi-deep wells in the region, cumulative decline of the Lenjanat aquifer level, increased electrical conductivity of water, and reduced recycled and in-basin water in the target and downstream area (Nekoabad dam to Abshar dam). The results show that institutional–governance, demand management, and executive–engineering solutions can help reduce these challenges. Management priorities in the Lenjanat area include managing changes in impervious surfaces (especially around the Zayandeh-Rood river and seasonal rivers), managing water use in the industrial sector, and controlling the number of wells (licensed and unlicensed); and in the Ben–Saman area, they include managing changes in cultivated areas.
